About the role
The role of the Level 1/2 Social Worker is to work within the Mission in Action of HammondCare to ensure a high standard of Social Work intervention is provided to the clients of our services, along with appropriate support to their carers. The Social Worker will participate as a member of the multi-disciplinary team to ensure a client centred, evidence-based, continuous quality improvement work environment, that is consistent with the values and ethics of HammondCare and the Australian Association of Social Workers (AASW).
The essence of this role:
- Provision of effective, timely and efficient social work interventions and service to the clients, their carers and families
- Provision of a holistic psychosocial perspective of the individual and their perspective, facilitating the client and their carers to identify issues, adapt and plan through the course of an admission, diagnosis or life changing event
- Participation as a member of the multi-disciplinary team in assessing the clients level of functional, care and support needs, setting achievable goals and contributing to appropriately meeting these needs and goals
- Participation as a member of the HammondCare team in the application of the organisations mission and values, to the pursuit of its objectives in a client centred, continuous quality improvement work environment.
About you

ESSENTIAL CRITERIA
- Recognised tertiary qualification in Social Work and eligibility for membership to the AASW. Qualifications obtained overseas must provide AASW Overseas Assessment number
- Current working rights for Australia i.e. citizenship, permanent residency or working visa
- Commitment to engage and align with HammondCares Mission in Action
- Minimum 2 years social work experience, desirable within a hospital or clinical setting
- Knowledge and skills in planning, implementation and evaluation of a range of psychosocial interventions, including psychosocial assessment, advocacy, problem solving etc. Complex discharge planning for clients with a wide range of health conditions is highly desirable
- Skills and experience in supportive counselling across a range of issues and client groups, e.g. adjustment to change, carer support, grief and loss etc
- Effective communication and interpersonal skills using a variety of media (eg. oral, written and computer)
- Self-motivated and skilled in effective time management, prioritising competing work commitments, and managing and responding to demanding situations
- Ability to interact constructively and collaboratively with the internal multidisciplinary team and external service providers
- Commitment to ongoing professional development, learning opportunities and professional supervision
- Current unencumbered Drivers Licence and safe driving record. Willingness to travel and work between HammondCares sites and services as required
DESIRABLE CRITERIA
- Theoretical and practical understanding of themes relevant to patients undergoing physical rehabilitation
- Experience providing Social Work Services in a Rehabilitation, Palliative Care or Aged Care Psychiatry setting
- Experience working in a multi-cultural environment
- Experience in the use of EMR system (NSW Health Electronic Medical Record) and clinical documentation.
