About the role
The role of the Level 1/2 Social Worker is to work within the Mission in Action of HammondCare to ensure a high standard of social work intervention is provided to the clients of our services, including appropriate support to their carers. The Social Worker will participate as a member of the multi-disciplinary team to ensure a client centred, evidence-based, continuous quality improvement work environment, consistent with the values and ethics of HammondCare and the Australian Association of Social Workers (AASW).
The essence of this role:
- Provision of effective and timely social work interventions and service to the clients, their carers and families
- Provision of a wholistic psychosocial perspective of the individual
- Participation as a member of the multi-disciplinary team in assessing the clients level of support needs and contributing to appropriately meeting their identified outcomes
- Participation as a member of the HammondCare team in the application of the organisations mission and values, to the pursuit of its objectives, in a client centred, continuous quality improvement work environment

ESSENTIAL CRITERIA
- Recognised tertiary qualification in Social Work and eligibility for membership to AASW. Overseas qualifications must provide AASW Overseas Assessment number
- Current working rights for Australia
- Minimum 1 year full-time post-graduate social work experience within a hospital setting
- Commitment to engage and align with HammondCares Mission in Action
- Skills and experience in planning, implementation and evaluation of a range of psychosocial interventions. Complex discharge planning for clients with a wide range of health conditions and social situations is desirable
- Skills and experience in supportive counselling across a range of issues and client groups
- Evidence of effective written and oral communication and interpersonal skills
- Self-motivated and skilled in effective time management and prioritising competing work commitments
- Ability to interact constructively and collaboratively with the multidisciplinary team and external service providers
- Willingness to travel and work between HammondCares sites and services as required. Current unencumbered Drivers Licence is desirable, with safe driving record
DESIRABLE CRITERIA
- Understanding of themes relevant to patients undergoing physical rehabilitation
- Experience working in a multi-cultural environment
- Experience in the use of EMR system (NSW Health Electronic Medical Record) and clinical documentation.
