In this role you will be responsible for providing a responsive and supportive social work service to consumers of Rural Northwest Health Community Health, Acute Care and Residential Aged Care areas.
Part time or Full time position available in Warracknabeal, Western Victoria.
About you:
- Demonstrated clinical practice experience.
- Ability to focus on building capacity of consumers to manage life challenges
- Proficient skills in psychosocial assessment, complex care and discharge planning
- Highly developed interpersonal and communication skills
- A progressive professional approach and sound multi-disciplinary team work skills
- COVID19 vaccination evidence
- Current Working with Children Check or willingness to obtain one
- Current and valid Police Clearance
- Current NDIS Check or willingness to obtain one
