About the role
Our Carmelite Health and Wellness team is seeking a Social Worker passionate about health and wellness for older adults, no matter their age, level of disease or disability. In this role, you will work in a person-centred, multi-disciplinary environment that embraces innovative practice and focuses on the reablement of our clients.
The key roles of the position are:
- Provide effective and efficient social work response to clients’ identified needs
- Assist clients to develop goals, review individual progress using self-assessment tools to achieve outcomes
- Provide counselling services to clients at an individual, couple or family level as appropriate
- Facilitate client access to and participation in community activities, establishing linkages that may be sustainable without ongoing support
- Liaise with other personnel involved in the care of the client to ensure optimal levels of care, including appropriate onward referral
- Establish and maintain partnerships with key community groups, service
providers and referral networks

About you
The successful candidate will demonstrate the following:
- Recognised social work degree (eligibility for full Australian Association of Social Workers membership)
- Previous experience as a qualified social worker
- Sound interpersonal and communication skills including demonstrated ability to work with older adults, their carers and families
- Current driver’s licence
- Understanding of loss and grief theories
- Knowledge of mental health interventions
- A basic knowledge of the requirements of the Aged Care Standards as are applicable to the role
