Our dynamic team is seeking to employ a dedicated, passionate, caring, and enthusiastic Support Coordinator on a contract basis.
Duties include:
- Monitoring and reviewing existing and new NDIS clients needs on regular basis.
- Assisting in goal planning and coordination of services
- Compiling, reviewing, and updating participant work orders.
- Ensuring participant budgets are adhered to.
- Monitoring participant care plans and end dates.
- Reporting as per government department and internal requirements.
- Working closely with our rostering team to schedule services.
Essential Requirements:
- Registered Social Worker, Psychologist, Registered Allied Health Professional, Health Sciences Professional, , Occupational Therapist, Developmental Educator
- Federal Police Check (or willingness to get)
- Working with Children Check (or willingness to get)
- NDIS Worker Screening (or willingness to get)
