ABOUT THE ROLE
We are currently looking for a Social Worker who is responsible for the delivery of quality care to patients across The Bays Healthcare Group by completing psychosocial assessments and intervention plans. In this role, you will be an integral part of the multi-disciplinary team and work closely with patients and their families or carers to assist in appropriately targeted interventions and safe discharge plan for inpatients.
The role requires approx. 15 hours of onsite work per fortnight. This role is available on a casual basis or as a contractor, with days and hours to be agreed with the successful candidate.
The key responsibilities of the role include:
- Complete assessments, which incorporate medical and psychosocial information
- Manage a variable workload to proactively identify psychosocial risk factors and prepare timely risk assessments and interventions
- Utilise a coordinated approach to patient care, which includes coordination and facilitation of family meetings as required and arranged with the relevant Unit Manager
- Consultation and an engagement with health professionals and the appropriate community agencies and providers
ABOUT YOU
The successful candidate will demonstrate the following skills, experience and attributes:
- Bachelor degree in Social Work and Registration as a Social Worker with Australian Association of Social Workers
- Minimum three years post registration experience within an acute hospital (or similar) and private hospital experience is preferred
- Ability to manage a case load independently and effectively prioritise tasks
- Highly developed interpersonal, practical negotiation, conflict resolution and communication skills.
- A good knowledge and ability to develop effective relationships with relevant community resources, information and service providers (Aged care providers, Local health service providers, etc)
- Competency with the My Aged Care portal
