About us

The Yarram and District Health Service (YDHS) is based in Yarram, 220 kilometres southeast of Melbourne on the South Gippsland Highway in rural Victoria. Yarram is 73 kilometres southwest of Sale and 62 kilometres southeast of Traralgon. 

Our geography places us 20 minutes to the beach and 2 hours from many of Victoria’s ski fields, which positions you in the perfect place for summer and winter day trips to some of the most picturesque landscapes in our state. 

The Yarram and District Health Service originally began operation as the Yarram and District Hospital in 1917, later changing its name in 1993 to reflect the multi-purpose facility it is today. YDHS provides our community with Acute inpatient services, including Dialysis, Urgent Care, Residential Aged Care, respite care, Primary Health, community services and an integrated GP Healthcare Centre. 

We have a strong focus on integrated service models providing seamless, streamlined healthcare, to the community which is critically supported by our skilled and dedicated staff group of approximately 180 people.

About the role

Working across the continuum of community, the Acute Ward and Residential Care, our Social Workers support consumers and their families through their health care journey. Within this role, the Social Worker provides high-quality, person-centred services to people at risk, and or experiencing homelessness. 

The Social Worker will use a Case Management Framework in order to help people who are homeless or at imminent risk of homeless.

About you

To be successful in this role, you’ll need:

Mandatory:

  • Hold a Masters/Bachelor’s degree in Social Work and be eligible for membership to the Australian Association of Social Workers. 
  • Demonstrate understanding of Social Work theory and the ability to relate it to practice. 
  • Ability to work constructively and co-operatively within a multidisciplinary team and with a wide range of external service providers.
  • Display efficient organisation, time management and prioritisation skills. 
  • Commitment to the YDHS values of care, compassion, respect, integrity, professionalism and safety.
  • Professional and effective behaviour, interpersonal and communication skills including computer literacy. 

Desirable:

  • Experience in healthcare or homelessness sectors.

Before commencement, the successful applicant will: 
Provide an original National Police Record Check no older than twelve (12) months, a Working with Children Check, apply for a NDIS Workers Screening Check, and complete a Pre-existing injury declaration form. All successful applicants will need to have had three (3) doses of the Covid-19 vaccine.
