JOB DESCRIPTION

Change Futures is a Brisbane based not-for-profit organisation providing Mental Health services to a range of community groups. Our mission is to work with people of all ages to provide opportunities for positive change and to enhance their psychological and emotional well-being.

We are looking for Social Workers and 4th or 5th year Psychology graduates for full time roles available now, working with older adults in Aged Care facilities in:

· Brisbane North

or

·Brisbane South

You will be responsible for supporting the delivery of our Psychology in Aged Care program, by providing assessment and psychological support to older adult clients. We prefer people to be living in the specific area they are working in as there is daily work related travel to a range of Aged Care facilities.

Key Responsibilities and Duties:

  • Provide evidence-based interventions to individuals/groups via face to face and telehealth
  • Maintain confidential session records and timely entry in client management systems
  • Compose detailed, concise correspondence to stakeholders (GP letters, risk reports)
  • Collaboratively liaise with agencies and allied health professionals
  • Provide mobile visits to clients
  • Contribute to team meetings and case conferences

Essential Criteria:

  • Completion of (or about to graduate) an approved degree in Social Work or Psychology
  • Eligible to apply for registration with either AASW or AHPRA 
  • Fully vaccinated in compliance with the Aged Care Industry
  • Unrestricted working rights within Australia
