BYFH is delighted and honored to be delivering the WA Home Stretch program, which gives young people who are leaving out of home care the choice to access ongoing levels of support until they reach the age of 21. We are looking for a ‘Staying On Facilitator’ to work within our Home Stretch team —someone with superb communication skills, case management experience and an understanding of Kimberley Aboriginal families, communities and cultural protocols. 

Our ideal candidate will be familiar with the experiences of young people living in out-of-home care. We want a person who can support—with enthusiasm and empathy—the transition of young people into adulthood and independent living.

Key Duties

  • Establish, coordinate and review Staying On agreements
  • Assist young people to connect with their support circles 
  • Facilitate intervention to manage conflict or issues in relation to living arrangements
  • Provide support for Housing Allowance Agreements and applications
  • Administrate and manage subsidies and reviews 
  • Work in close collaboration with district placement officers and Child Safety Officers for smooth transition of carer support
  • Liaise with subsidy payment unit to offer oversight of subsidy payments to carers
  • Complete core training with Home Stretch Community of Practice
  • Participate in Home Stretch team’s Knowledge Hub for reflective practice discussions 
  • Contribute to shared practice knowledge across network

Essential Criteria:

  • Qualification in Bachelor or Diploma in a relevant field is preferred; however unqualified applicants with demonstrated significant relevant experience (minimum 2 years) and skills working with young people with experience of out of home care will be considered.
  • Understanding of Kimberley Aboriginal families, communities and cultural protocols
  • Case Management experience
  • Extensive knowledge of issues affecting parents / carers and children as well as youth
  • Demonstrated experience working with parents / carers and children / young people
  • Understanding of community development principles
  • Demonstrated experience planning, implementing and evaluating programs and activities
  • Previous experience working with Aboriginal clients
  • Knowledge of administration, filing and reporting systems
  • Excellent written and verbal communication skills
  • Ability to work in a team and to work collaboratively with others
