Sunflower Services, an emerging NDIS provider with a focus on Newcastle, NSW, and Adelaide, South Australia, is seeking a dedicated individual to join our team as a Support Coordinator/Psychosocial Recovery Coach. As a boutique NDIS provider specializing in Complex & Forensic Mental Health Support, Supportive and Youth Justice, we are committed to delivering services through a trauma-informed lens. Position: Support Coordinator/Psychosocial Recovery Coach

Location: Adelaide, South Australia

Job Type: Permanent Full time (potential part time)

Responsibilities:

  • Utilize your experience as a Support Coordinator or Psychosocial Recovery Coach to work autonomously and meet essential KPIs, either from home or in-office.
  • Demonstrate a person-centred approach with a deep understanding of Trauma-Informed Care.
  • Efficiently multitask and work therapeutically with participants, developing strategies and positive habits for implementation in their daily lives.
  • Create Recovery Plans for participants with Psychosocial disabilities.
  • Collaborate with families and stakeholders to discuss progress and strategies for participants.
  • Advocate the belief that individuals are the experts in their own lives.
  • Assist participants in exploring various services and supports, aligning with the principles of Choice and Control.
  • Foster partnerships with clinical mental health services, allied health professionals, and other relevant stakeholders.
  • Maintain detailed progress and case notes, identifying and proactively managing potential crisis situations for complex participants.
  • Develop risk assessments for participants facing complex challenges.
  • Prepare plan reviews and other reports requested by the NDIA.
  • Educate clients on understanding the NDIS and adapt to diverse client journeys and complex behaviours.
  • Coordinate a range of supports, including informal, mainstream, and funded providers, ensuring participants complete service agreements where necessary.
  • Monitor plan outcomes and expenditure regularly to stay within the plan budget.
  • Market Sunflower Services and our ethos to other service providers and Local Area Coordinators.
  • Collaborate with government services such as the Department of Child Protections, Exceptional Needs Unit, and Department of Health Services to provide support within the NDIS framework.
  • Work alongside the Specialist Support Coordinator to deliver high-standard support for complex participants.
