Support Coordination/ Psychosocial Recovery Coach

Job Title: Sunflower Services Support Coordinator / Psychosocial Recovery Coach

Job Overview: Sunflower Services, a leading NDIS mental health  organisation, is seeking a dedicated and compassionate Support Coordinator / Psychosocial Recovery Coach to join our dynamic team. This unique role blends elements of coordination and coaching to provide comprehensive support and guidance to individuals on their journey to psychosocial recovery. As a Support Coordinator / Psychosocial Recovery Coach, you will play a pivotal role in helping individuals enhance their overall well-being, develop coping strategies, and achieve their personal goals.

Key Responsibilities:

Client Assessment and Planning:

  • Conduct thorough assessments of clients’ psychosocial needs, strengths, and challenges.
  • Collaborate with clients to develop personalised recovery plans that align with their NDIS goals and aspirations.
  • Continuously monitor and evaluate progress, adjusting plans as necessary.
  • Engage in NDIS plan re-assessments, plan reviews, and AAT tribunal meetings. 
  • Monitor, assess and connect the individuals NDIS plan. 
  • Effective budgeting and numerical skills is important. 

Psychosocial Support and Coaching: 

  • Provide empathetic and non-judgmental coaching to clients, utilising evidence-based techniques and interventions.
  • Offer emotional support, active listening, and guidance to individuals facing mental health challenges.
  • Help clients develop effective coping mechanisms and resilience skills.
  • Create a Recovery Plan and Report and submit it to the NDIS. 

Coordination and Resource Referral:

  • Coordinate with external service providers, healthcare professionals, and community resources to ensure holistic support for clients. 
  • Assist clients in accessing appropriate social services, housing, employment opportunities, and other necessary resources.
  • Coordinate a range of mainstream and non mainstream supports. 

Skill Building and Coaching:

  • Facilitate individual and group coaching sessions to enhance clients’ life skills, self-esteem, and overall psychosocial functioning.
  • Collaborate with clients to set achievable goals and support their progress towards personal growth and recovery.

Advocacy and Empowerment:

  • Advocate for clients’ rights and needs within the organisation and in the larger community.
  • Empower clients to become active participants in their own recovery process and decision-making.

Documentation and Reporting:

  • Maintain accurate and up-to-date client records, progress notes, and documentation of interventions.
  • Prepare regular reports and summaries of client progress for internal and external stakeholders.

Professional Development:

  • Stay current with best practices, research, and trends in the field of psychosocial recovery and the NDIS. 
  • Participate in ongoing training, workshops, and skill development to enhance coaching techniques.
  • Meet daily and weekly KPI targets efficiently. 

Qualifications:

  • Minimum Diploma in Community Services, Mental Health, Alcohol & other Drugs or Youth Work. 
  • Relevant certifications or training in Social Services related fields. 
  • Proven 1 year experience in Support Coordination and/ or Psychosocial Recovery Coaching. 
  • Strong interpersonal skills with the ability to establish rapport, build trust, and maintain boundaries.
  • Excellent communication skills, both verbal and written.
  • Knowledge of community resources and social services.
  • Empathy, compassion, and a non-judgmental attitude.
  • Ability to work collaboratively within a multidisciplinary team.
  • Proficiency in documentation and record-keeping.
  • Ethical and culturally sensitive approach to client care.
