Role Overview:

Support Coordinator:

To support people with psychosocial disability to participate more fully in the community and enhance their capacity for social and economic participation through:

  • Strengthening their ability to coordinate and implement supports in their National Disability Insurance
  • Scheme (NDIS) individualised funding packages
  • Supporting their engagement with mainstream, formal and informal services
  • Developing capacity and resilience in their network.

 Recovery Coach:

  • Developing a recovery-enabling relationship with people including their family and carers
  • Supporting a person to develop and implement their individual recovery plan
  • Coaching a person to increase recovery skills and personal capacity, including motivation, strengths, resilience, and decision-making
  • Collaborating with broader systems including available services and resources
  • Supporting people to become active participants in the NDIS

 

Requirements:

  • Minimum of a Cert IV in a Community services related discipline (ideally a completed degree or Diploma)
  • Experience in a human services environment, particularly experience in understanding service delivery and how it can best deliver for clients with persistent and complex mental health.
  • Experience in assisting participants to test their eligibility of the National Disability Insurance Scheme (NDIS) or transition them to other appropriate supports.
  • Highly-developed communication, interpersonal and negotiation skills including the ability to work and communicate effectively with clients, their families, community agencies and other professionals.
  • Ability to meet deadlines in a high demand work environment and to exercise judgment and initiative.
  • Excellent organizational and time management skills.
  • Must be eligible to apply/currently hold a Blue Card
  • Must hold/be willing to apply for NDIS worker screening clearance 
  • Current Drivers License
