About the role
- Work collaboratively with people, their families, carers, allied health professionals and service providers to design and implement an individualised plan.
- Provide informed choice and control with a person-centred, action-based approach.
- Facilitate utilising relevant and appropriate services and supports to achieve established and evolving goals.
Required:
- Excellent interpersonal skills with the ability to actively listen and build rapport.
- Ability to work within a team and with other organisations.
- Time management and administrative skills – multi-task and prioritise work.
- Planning and problem-solving skills.
- The ability to write accurate and concise case notes and draft reports.
- Cert IV level or above in Community Services, Disability Services, Mental Health or equivalent.
- Open Drivers Licence (Travel to surrounding suburbs, including Nowra)
- NDIS Worker Orientation (completed online in approximately 1 hour)
- Working with Children’s Clearance
- NDIS Worker Screening Check (may be applied for at the post-interview stage).
Desirable:
- Degree in Social Work or similar field
- Experience working in similar roles
- Experience with mental health issues is highly regarded
- Understanding the NDIS (National Disability Insurance Scheme) and other government services

Essential:
