About the role 

  • Work collaboratively with people, their families, carers, allied health professionals and service providers to design and implement an individualised plan.
  • Provide informed choice and control with a person-centred, action-based approach.
  • Facilitate utilising relevant and appropriate services and supports to achieve established and evolving goals.

Required:

  • Excellent interpersonal skills with the ability to actively listen and build rapport.
  • Ability to work within a team and with other organisations.
  • Time management and administrative skills – multi-task and prioritise work.
  • Planning and problem-solving skills.
  • The ability to write accurate and concise case notes and draft reports.
  • Cert IV level or above in Community Services, Disability Services, Mental Health or equivalent.
  • Open Drivers Licence (Travel to surrounding suburbs, including Nowra)
  • NDIS Worker Orientation (completed online in approximately 1 hour)
  • Working with Children’s Clearance
  • NDIS Worker Screening Check (may be applied for at the post-interview stage).

Desirable:

  • Degree in Social Work or similar field
  • Experience working in similar roles
  • Experience with mental health issues is highly regarded
  • Understanding the NDIS (National Disability Insurance Scheme) and other government services

Essential:

  • COVID-19 Vaccination certificate, PPE and Infection Control Training (in accordance with Government Health Mandates).
  • NDIS Worker Screening Check (may be applied for at the post-interview stage).
