About the position

The Way Back Support Service focuses on providing support for up to three months in the critical period following a suicide attempt or suicidal crisis. The service provides non-clinical, practical support and linkages to services, as well as peer support, in the community around their identified needs of support. 

 
The Way Back Support Service are looking for a passionate and motivated Support Coordinator to work in a multi-disciplinary team, providing non-clinical psychosocial support to people experiencing a suicidal crisis or who have attempted suicide and their families. Support targets those at the highest risk through referrals following hospital presentations or through specialist mental health services.
 
Support Coordinators work collaboratively with consumers within a recovery and strengths-based approach to formulate, implement and review support plans, needs assessment and safety plans. 
 
Through respectful and meaningful engagement, they work alongside people to develop a plan to reduce their drivers of suicidality by also linking into other key services
 
Based in our Campbelltown hub, this is a part-time (3 days per week), fixed term position until 30 June 2024, with likelihood of extension. Hours and days are negotiable. GPH offers a great place to work with a commitment to flexibility, innovation, and teamwork.

We are looking for someone with:
 
  1. Relevant qualifications (minimum Cert IV or other relevant qualification in mental health or human services), and/or experience in suicide prevention or mental health work.
  2. Sound understanding of the personal, family, and social issues associated with suicidal ideation and attempts, including awareness of groups identified to be at increased risk of suicide.
  3. Demonstrated case management skills and ability to work effectively with clients, family members and service providers to identify solutions.
  4. Excellent communication skills, particularly with individuals and their families/carers who may be experiencing significant distress.
