St John of God Social Outreach provides a range of no or low-cost mental health, homelessness and capacity building services to over 5500 people in the community across Victoria, Western Australia, New South Wales and the Asia Pacific, particularly in areas where we have identified gaps in service provision.
The Position
Horizon House Young Mother and Baby Program Dianella provides accommodation, holistic care, and intensive support to pregnant young women aged 16 to 22 years who are vulnerable and homeless or at risk of homelessness. Horizon House Dianella provides support to residents during pregnancy, preparing for the birth of their child, post-natal follow up, developing their independent living skills and assisting their transition to independent living.
A rare opportunity is available for a Support Coordinator to join our friendly and committed team in the Young Mother and Baby Program at our Horizon House Dianella.
In the position of Support Coordinator you will primarily oversee and coordinate the operational running and program delivery of the YMBP at Horizon House Dianella, including associated transition placements. This entails working in collaboration with the Practice Lead to prepare and assess new referrals, facilitate regular team meetings including case discussions and service coordination. You will also provide line management of team members including regular supervision, performance development and individual de-briefing as required.
To be considered Applicants must hold a minimum Certificate 4 in Youth Work, Social Work or Psychology. Extensive experience working with young mothers and babies in a residential care setting with an understanding of early development needs is essential for this position.

This is a permanent full time position of 76 hours per fortnight.
The successful candidate will have:
- A willingness and capacity to embrace the Mission and Values of St John of God Health Care.
- Tertiary qualification from a recognised University and/or TAFE College in Youth Work, Social Work or Psychology and a minimum of 5 years’ experience working with young people in a residential care setting.
- Experience of working with Mothers and Babies with an understanding of early development needs.
- Well-developed understanding of trauma informed care including theoretical perspectives such as loss and grief, attachment, resilience and connectedness, child and adolescent development and the capacity to apply a range of trauma informed practice responses based on these theories.
- Sound understanding of the particular challenges involved in service delivery to young people in the homelessness sector.
- Experience in program coordination and leadership of a team to successfully support program delivery.
- High level of verbal and written communication skills and computer literacy.
- Demonstrated ability to take initiative, work independently, and as a team
- Strong risk management skills with effective, flexible and sound problem solving skills.
- Participate in the out of hour’s roster as On Call Coordinator.
- Hold a full (unrestricted) State Drivers License.
- Be responsible for maintaining current First Aid and CPR Certification.
