About the role

Family Spirit have an exciting opportunity for an experienced case manager to join our Permanency Support Program (PSP). In this role you will work closely with the children and young people, carers and birth families to provide high quality and effective case management services within a therapeutic and trauma informed model. You will be responsible for:

  • Developing and implementing therapeutic case plans for children and young people in long term out of home care.
  • Work with Children and or Young People to restore them to the family through a trauma informed case plan.  
  • Assist with permanency outcomes for children and young people through Adoption or Guardianship pathways. 
  • Engaging with children, young people, families and carers, and undertake assessments to identify their support needs. 
  • Support the development of Cultural Plans
  • Assist with the development and implementation of Positive Behavioral Support Plans. 
  • Adhering to mandated reporting obligations and legislative requirements
  • Maintaining accurate records and documentation
  • Willingness to participate in an agency on call roster.

About you

  • A minimum of Diploma of Community Services, Youth Work or Counselling.  Preference is for a Degree Social Science, Teaching, Policing, Nursing or Psychology.  
  • Aboriginality is considered as a suitable qualification to practice as an identified Case Worker.   
  • Experience with case work practice. 
  • Understanding of the Child Protection legislation
  • Understanding of trauma informed care
  • Sound interpersonal skills including ability to work collaboratively with multi-disciplinary team
  • Strong time management skills 
  • Proficiency in using computer applications including Microsoft Office and client/data management systems
  • Current Working with Children Check clearance
  • Current and valid driver’s licence with access to a well-maintained vehicle which will be used for work purposes.
