About the service
The Alcohol and Other Drug (AOD) Transition Support Service provides a client centred, stepped care model offering individual and group counselling, pre- and post-rehabilitation support, relapse prevention, case management, group programs, care planning and care coordination for clients with AOD dependence, mental health dual diagnosis and complex needs and their families and carers.
About the role
Working with the Cairns Withdrawal Support Service and Shanty Creek Therapeutic Community, the Transition Wellbeing Worker/Case Manager will deliver individual and group-based counselling and support services to proactively assist and support clients with entry and exit into suitable residential treatment.
The Transition Wellbeing Worker /Case Manager will deliver their role as outreach support and will work closely with various residential treatment facilities to enable and support the client in and out of these treatment settings in a way that maximises person-centred care in an integrated, supportive and professional manner. Duties include:
- Provide high quality engagement, assessment, counselling and support services to clients, using evidenced based practice principles
- Manage a caseload of clients, through providing support, service coordination, referral and advocacy to assist client in achieving their goals
- Effectively assess and manage client risk
- Effectively support clients in developing and implementing holistic client service plans that are considerate of a range of client needs including: personal medical/treatment, spiritual, values/beliefs, vocational, social/relational, recreation/leisure, physical, financial, housing/environment
- Develop, facilitate and evaluate therapeutic group programs aimed at achieving client readiness for AOD residential treatment
- Actively support client readiness to support successful attendance and completion of residential treatment
- Develop, facilitate and evaluate therapeutic group programs at aimed at building client capacity and confidence in exit from residential treatment
This is a full time ongoing role, working 38 hours per week.
About you
You are an engaging individual with the ability to connect with clients who are navigating difficult life experiences. You can work both autonomously and collaboratively within a team and utilise the appropriate techniques to support your clients. You will also demonstrate the following:
- Demonstrated experience in working with clients who have AOD issues/concerns
- Applied knowledge in contemporary evidence based AOD counselling practice, case management and intake and assessment
- Demonstrated experience in facilitating groups
- Demonstrated experience in working autonomously – in particular, managing competing work demands and your own work schedule
- Experience in a role where you have successfully communicated across various work teams
- Highly developed communication and community engagement skills
The base salary range for this role is $88,761.92 to $92,773.20 per annum, dependent on skills and experience.
