The Opportunity
IPC Health have partnered with Western Health and the Royal Children’s Hospital to co-design and lead the Brimbank Melton Infant Child and Family Health and Wellbeing Hub!
The Hub will provide a ‘one-stop-shop’ supporting infants, children and families, delivering services like psychology and psychiatry, allied health, family supports and paediatric care. People with lived experience will also form an important part of the Hub’s workforce. The service will support children aged 0-11 who are experiencing developmental, behavioural or emotional challenges and their families, and will focus on early intervention and providing age-appropriate care.
Based at a new site in Brimbank or Melton, the Hub will offer a holistic and family centred specialist intake assessment including autism and developmental assessments, helping families identify their needs and access the right services within the Hub, or with partner community health services or family services in the broader service system.
The Hub will not just treat, but also aim to build family capacity and capability!
Wellbeing Coordinators will play an integral part in this Hub delivering family centred, holistic and timely care within a multi-disciplinary team.
With your clinical expertise and experience in conducting holistic wellbeing assessments, you will engage with families who may be experiencing disadvantage or life challenges to achieve their wellbeing goals. You will be key to supporting families as they access services in the Hub, from entry to exit; care planning, capacity building through a strengths based approach to empower families and safety and risk assessments. With a continuous quality improvement focus, you will be part of the co-design process of the Hub and collaborate with a variety of services to improve social, emotional and physical determinants of health of infants, children and families.
This opportunity is seeking 2FTE Ongoing positions with flexibility to consider Part Time with a minimum 0.4FTE commitment. The role is classified under either enterprise agreement depending on your relevant field and qualification
- Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi Enterprise Agreement 2017.
- Victorian Stand Alone Community Health Centres, Health Professionals Multi-Employer Agreement 2017-2021
- Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2020-2024
You will make a difference by
- Delivering holistic family centred care that responds to the changing needs of children and families, empowers families experiencing adversity to recognise their strengths, aspirations and wellbeing goals.
- Facilitate smooth transitions and meaningful connections with other Practitioners that help clients achieve their goals.
- Establishing strong relationships with key internal and external stakeholders to continuously improve referral pathways, accessibility and service collaboration.
- Actively participating in the co-design, implementation and scale up of the Infant, Child and Family Health and Wellbeing Hub.
To succeed, you will need
- A tertiary qualification and 4 years’ experience in the following fields: nursing, social work, allied health, counselling, or community development.
- Proven experience and knowledge in applying a strength based approach; effective motivational interviewing and holistic wellbeing assessments with diverse families and disadvantaged communities.
- Demonstrated experience helping families to navigate service systems, while providing support from entry to exit.
- A current and valid Victorian drivers licence and Working with Children’s Check.
