Position

As the Wellbeing Facilitator, you will be responsible for the development, implementation, evaluation, and continuous improvement of lifestyle programmes within this area of expertise. Supporting the overall Community, Culture, & Wellbeing (CC&W) department, you will be part of delivering a person-centred life-enriching program, using a best practice approach to meet our residents’ lifestyle needs across the spectrum of recreation, culture, and wellbeing.

 

KEY RESPONSIBILITIES

  • Assessment and review of residents’ needs: Using a person-centred approach, identify and document resident lifestyle needs using electronic assessment and care plan tools and documents in iCare to comply with Aged Care Standards and Emmy Monash Policies and Procedures
  • Develop and review care plans in consultation with resident and/or family to meet ongoing needs of residents 
  • Develop program objectives and content for programs for individuals and groups
  • Facilitate connections between residents through one-on-one and group settings

 

Key Selection Criteria

  • Certificate IV in Lifestyle and Leisure or Allied Health is a minimum requirement
  • Proven experience in Aged Care, preferably within a lifestyle, recreational, or social program setting
  • Ability to work autonomously and within a team
  • Natural flair for creativity and building strong relationships with residents
  • Working experience in a dementia setting is highly desirable
  • Sound interpersonal and customer service skills
  • Good computer literacy using MS Office and Outlook
  • Provide evidence of ability to work legally in Australia
