Wellbeing Manager – (Clinical Case Management ) Feros Care

JOB DESCRIPTION

We are excited to be recruiting for a Wellbeing Manager to apply your clinical skillset to case manage your own portfolio of clients through their wellbeing journey. You will split your time between working from home and within the community of Byron Bay/Ballina regions.

This is a full time role offered on a fixed term contract (with the possibility of extension based on funding). Your journey with Feros Care will commence with a comprehensive training and orientation session to support you in your new role.

About the role:

  • Help your clients to remain at the centre of their journey
  • Develop STRC/HCP care plans and coordinate a multi-disciplinary team (MDT) for the delivery of in-home therapy, services, and equipment
  • Manage & Coordinate the day to day delivery of care for STRC/HCP clients including managing their individualized budgets.
  • Be the advocate for the client and participate in case conferencing and scheduled clinical appointments as required.
  • Initiate linking service support for clients identified as having complexities or vulnerabilities that prevent access to mainstream services which could assist them to live in their community with dignity, safety and independence.

About you:

  • Tertiary qualifications in Nursing or Allied Health (with relevant registration)
  • Previous Case Management Experience, with a clinical focus (Community and Aged Care experience highly desired)
  • Demonstrated ability to identify and prioritise complex problems and issues, generate solutions and work collaboratively to achieve a successful outcome.
  • Knowledge of home care packages or similar Commonwealth funding packages with a clinical focus is desired
  • Experience working with clients conducting STRC/HCP assessments and applying goal-oriented, outcome focused and holistic approaches
  • Knowledge and understanding of Risk Management principles, practices and legislation including Work, Health and Safety and Injury Management
  • Demonstrated ability to develop and maintain strong networks with internal and external stakeholders
  • Current drivers license and be comfortable traveling throughout the region each day
  • COVID-19 Vaccination ( NSW, 2 + 1 booster )
