Rise is on the hunt for a Case Worker to take responsibility of the ‘Keny Moort Program’ in collaboration with Indigo Junction and Swan City Youth Services. The primary role will be to work in a case management capacity with young people at risk and assist in the reduction of criminal and anti-social behaviours in the Midland area through the delivery of educational and vocational programs. These include but are not limited to Keys for Life and Stopping Family Violence. Additionally, part of the role will include supporting other youth services at Rise.
A typical day may look like……
- Program development including coordinating/facilitating ongoing early intervention diversional and recreational activities for the young people attending the Stratton Edge Youth Centre and developing support and action plans.
- Establishing and maintaining links with relevant community-based organisations
- Consumer services and coordination including resolving complaints and actioning compliments promptly
- Contributing to recruitment, selection, and development of staff in the team
- Program administration including maintaining accurate records/documentation, monitoring compliance, and contributing to reports.
This position will be full-time working Monday to Friday – 8:30 to 4:30 with some flexibility required to meet client needs. Your working locations will vary across: Rise Helen’s Place office (Middle Swan), Stratton Community Centre (Middle Swan) Indigo Junction (Midland), East Metro community and work-from-home.

- A qualification in a relevant human services area or matching experience
- A Certificate IV in Training and Assessment
- Understanding of the principles of Trauma Informed Care
- Demonstrated knowledge and understanding of the complex issues impacting young people at risk, their families, friends, advocates, and others
- Ability to organise and facilitate programs and events for youths at risk
- First aid certificate
- Current ‘C’ class driver’s licence

Appointments to this position will be subject to a NDIS worker screening check, National police clearance and working with children check. You must also hold a Current ‘C’ class driver’s licence. Costs for new checks will be covered and/or reimbursed by Rise.
