Where you’ll be working

Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. What you’ll be doing

The Youth Health Worker works within a multidisciplinary team and provides services for young people aged 12- 24 years and their significant others. WSLHD Youth Health services prioritise marginalised, homeless and at risk young people aged 12- 24 years. This position will facilitate engagement by providing services that are relevant and responsive to the health needs of the target group. This involves the provision of information, assertive case management, support, group work, health education, brief intervention and referral to appropriate services both internal and external to WSLHD Youth Health.

The youth health worker provides clients with a consistent staff member who can actively facilitate and coordinate service access and care navigation within a primary health care (PHC) framework.

Essential Criteria

Qualification: Tertiary qualifications in relevant discipline

Selection Criteria

  1. Qualifications and/or experience in a relevant field. And Valid NSW drivers licence.
  2. Experience working with and understanding of the issues affecting marginalised young people’s access to health services.
  3. Demonstrated experience in planning, delivering and evaluating health promotion activities and/or community development projects.
  4. Knowledge of the health issues that impact on marginalised and at risk young people aged 12- 24 yrs.
  5. Excellent communication and networking skills.
  6. Demonstrated ability to work as part of a multidisciplinary team.
  7. Excellent computer skills with experience in a range of Microsoft applications.
  8. Demonstrated ability to engage with marginalised and at risk young people both individually and in a group setting.
