Remuneration: $67,357.32 – $125,151.00 per annum, pro rata (+ super + 17.5% leave loading where applicable)

Our District Hospital is a 34 bed facility providing emergency, general ward, surgical, obstetric and radiology services.  Community health services include community nursing, generalist counselling, physiotherapy, mental health and transitional aged care. Additional services provided by visiting staff include speech pathology, dietetics and occupational therapy.

About the Role   

The focus is on young people with complex or high needs who may be difficult to engage in effective treatment and support. The aim is to guide the future management of these young people in primary health settings, provide evidence based interventions or to facilitate referral to specialist settings where required. These young people may be exhibiting behaviour which is indicative of a developing mental health problem, i.e. prodromal symptoms that may be recognised by parents, teachers or other service providers, e.g. Drug and Alcohol services.  

Identified outcomes are:

  • Successful therapeutic engagement with young people experiencing or at risk of developing severe mental illness, who may be considered very hard to engage
  • better experience of care through integrated and coordinated service models (with a particular focus on headspace centres, child and adolescent mental health services, hospitals and schools)
  • avoidance of unnecessary hospitalisations through timely access to appropriate and targeted clinical care
  • active use of a multiagency care plan for young people with, or at risk of, severe and complex mental illness, to help link providers across multiple services involved in a young person’s care
  • improved support for families and carers of young people experiencing severe mental illness

Priority populations will include young people who identify and Aboriginal and/or Torres Strait Islander, young people from culturally and linguistically diverse backgrounds, young people experiencing social and/or economic disadvantage, young people at risk of withdrawing from education or employment, young people who have experienced trauma, and young people who are at risk of hospitalisation.
