Key Responsibilities:
Individual Support and Case Management:
- Provide one-on-one support to young people dealing with various life challenges, such as mental health issues, substance abuse, family problems, homelessness, or educational challenges.
- Develop personalized support plans based on individual needs and circumstances.
- Assist with goal setting and provide resources to help achieve those goals (e.g., housing, education, employment).
Advocacy and Referrals:
- Advocate for youth in accessing appropriate services, such as counseling, legal support, education, and health services.
- Make referrals to specialists and other community services, ensuring a coordinated approach to meeting the needs of the young person.
Program Development and Delivery:
- Design and implement programs aimed at supporting youth in areas such as life skills, career readiness, and mental health.
- Lead group workshops, educational sessions, or outreach initiatives to raise awareness on important issues affecting youth.
Crisis Intervention:
- Provide immediate support in crisis situations, including emotional support, safety planning, and assistance with accessing emergency services.
Collaboration:
- Work with other professionals, including stakeholders, guadians, counselors, social workers, and health providers, to ensure comprehensive support for young people.
- Participate in community meetings and contribute to the development of policies or programs aimed at improving youth services.
Monitoring and Reporting:
- Keep detailed records of interactions, progress, and outcomes related to youth support.
- Prepare reports for management or funding agencies, tracking service delivery and youth outcomes.
Community Engagement:
- Build relationships with local community organizations and businesses to create opportunities and support for youth.
- Engage with youth in various settings, including schools, community centers, and social media, to understand their needs and provide relevant support.
Skills and Qualifications:
- Bachelor’s degree in social work, psychology, education, or a related field (some positions may require additional certifications).
- Experience working with youth in a support or case management role.
- Strong interpersonal and communication skills to effectively engage with young people and professionals.
- Knowledge of community resources and services available to youth.
- Ability to handle challenging situations and provide crisis intervention when necessary.
- Empathy, patience, and a non-judgmental attitude toward youth facing diverse challenges.
Work Environment:
- Work settings includes client sites, community centers, residential care facilities, and office environments.
- Some travel may be required to attend meetings, visit youth in their homes, or liaise with community organizations.
