THE ROLE – This role works toward providing young people (aged 15+9mths to 21) skills and networks to be able to transition into adulthood successfully from the out-of-home care setting:
- Case manage a number of young people who are transitioning from care.
- Undertake assertive outreach and support that includes varying levels of intensity
- Motivate, encourage and empower clients to address areas of personal difficulty.
- Actively support, encourage and maintain the development of positive relationships, networks and linkages within the young person’s community.
- Actively promote the establishment and maintenance of positive family Relationships.

THE IDEAL CANDIDATE – what you will bring;
- Working knowledge of the Child protection system and the complexities of young people transitioning from care.
- A demonstrated understanding of the social and political issues impacting young people.
- Demonstrated experience and knowledge of case management framework and practices.
- Valid Victorian Driver’s Licence |Working With Children Check |Police Check
- Minimum requirement of a Diploma of Community Services or other relevant tertiary qualifications and/or experience appropriate to the role.
