Youth Transition Advisor – Better Futures (East Gippsland) Quantum Support Services

JOB DESCRIPTION

THE ROLE – This role works toward providing young people (aged 15+9mths to 21) skills and networks to be able to transition into adulthood successfully from the out-of-home care setting: 

  • Case manage a number of young people who are transitioning from care. 
  • Undertake assertive outreach and support that includes varying levels of intensity  
  • Motivate, encourage and empower clients to address areas of personal difficulty. 
  • Actively support, encourage and maintain the development of positive relationships, networks and linkages within the young person’s community. 
  • Actively promote the establishment and maintenance of positive family Relationships.

  • Salary – SCHADS Award Level 5| $93,850 P/A or Pro-rata for 0.6EFT position | + Superannuation | Annual indexation | Portable long service leave | Leave opportunities.

THE IDEAL CANDIDATE – what you will bring;

  • Working knowledge of the Child protection system and the complexities of young people transitioning from care. 
  • A demonstrated understanding of the social and political issues impacting young people. 
  • Demonstrated experience and knowledge of case management framework and practices. 
  • Valid Victorian Driver’s Licence |Working With Children Check |Police Check 
  • Minimum requirement of a Diploma of Community Services or other relevant tertiary qualifications and/or experience appropriate to the role.
